New therapies in the management of psoriasis: what evidence is available?

Abstract

Psoriasis is a chronic inflammatory disease affecting 60 million people worldwide, with a multisystemic impact that includes cardiovascular and metabolic comorbidity. Its multifactorial etiology involves genetic predisposition and environmental factors, highlighting the IL-23/Th17 axis as key in its pathophysiology. We conducted a narrative review exploring therapeutic advances by searching databases Medline, PubMed, EMBASE, Cochrane Library, OVID and ClinicalKey. Original studies and reviews on efficacy, safety and mechanisms of treatments in psoriasis were included. New therapeutic agents, such as bimekizumab, a dual IL-17A/F inhibitor, and tapinarof, an AhR modulator, have demonstrated significant improvements in PASI90 and PASI100. In addition, roflumilast and apremilast offer safe profiles in topical and systemic application, respectively, improving patient quality of life. The evidence supports the effectiveness of innovative therapies in psoriasis, highlighting the need for long-term research to personalize treatments and address socioeconomic issues.
