We present the case of a 2-month-old female patient admitted to the emergency department with a diagnosis of community- acquired pneumonia, with severe hypokalemia and metabolic alkalosis associated with the use of piperacillin-tazobactam. 48 hours after suspending the latter, he presented improvement and normalization of potassium values. The diagnosis of severe hypokalemia and metabolic alkalosis associated with this drug was made, in this case, after having excluded other causes. The presence of hypokalemia is a severe adverse effect that can occur in patients who are being managed with piperacillin-tazobactam, so it must be taken into consideration with the use of this antibiotic.
