Diabetic ketoacidosis is a serious complication of relative or absolute insulin deficiency that primarily affects type I diabetes. The classic clinical triad of polydipsia, polyuria, and weight loss in the setting of a patient with tachypnea and altered consciousness should lead to suspicion of this entity, however, the clinical data, especially in infants, can be atypical. We present the case of a 14-month-old male infant who attended the pediatric emergency room with low-grade fever, vomiting, gingivorrhagia, tachypnea, and altered level of consciousness who was diagnosed with severe diabetic ketoacidosis.
