Arthroscopy is useful for the management of ankle fractures. We present a 49-year-old man with an ankle fracture plus ligament, chondral, and syndesmotic injuries diagnosed and treated with the aid of arthroscopy. The patient evolved favorably, returning to sports at 8 months. Although there is no agreed criteria on the use of arthroscopy in ankle fractures, it is a valuable tool for the restitution of joint surfaces and the verification of ligament instabilities. It is necessary to detect trauma mechanisms associated with hidden injuries that will benefit from arthroscopic management.
