The giant congenital nevus is a disease that can presence melanoma, which can be fatal for the patient’s prognosis. This pathology has both an aesthetic and an emotional component due to the physical characteristics of the nevus. We present a surgical technique for a major facial reconstruction of a giant congenital nevus in a patient with data suggestive of the onset of malignancy. In this technical report, the reconstructive technique is exposed. It is concluded that by performing an adequate pre-surgical plan, reconstructions of major facial defects with loco-regional flaps and the use of a skin expander can be performed.
